Key Responsibilities\n

• Guide primary care providers in determining appropriate treatment routes, differentiating between cases suitable for primary care and those requiring referral to specialized mental health resources.

• Initiate and monitor follow-up procedures to assess patient progress and make timely adjustments to treatment strategies.

• Develop comprehensive relapse prevention plans for patients, aiding them in maintaining stable functioning throughout their treatment journey.

• Identify at-risk patients and design proactive intervention plans to mitigate the deterioration of their mental health conditions.

• Coordinate the delivery of healthcare services in behavioral health, fostering communication connections both within the primary care environment and with external services when necessary.

• Maintain accurate records of patient progress and diagnostic information as part of the treatment chart to ensure continuity of care.

• Engage in transparent communication with primary care providers to effectively keep them informed about patient needs and progress, while collaborating on treatment plans.

• Work with patients to facilitate behavioral changes, especially those with co-occurring physical conditions, promoting healthier lifestyle choices.

• Provide patients with self-management tools and educational resources to enhance their active engagement in treatment and recovery processes.

• Offer consulting insights to clinic management regarding behavioral services, along with suggestions for evaluating outcomes and assessing program efficacy.

Qualifications\n

Compensation and Benefits\n

This full-time position offers a competitive salary range between USD 87,000 and 105,000 annually, based on experience and qualifications.
