Key Responsibilities\n

• Facilitate comprehensive patient assessments, focusing on diagnostic and functional evaluations to inform treatment plans.

• Collaborate closely with primary care providers, identifying and addressing mental health and psychosocial issues.

• Develop and implement targeted interventions, ensuring that patients receive the appropriate mental health services based on their individual needs.

• Assist primary care providers in determining the suitability of patients for various treatment pathways, including referrals to specialized facilities.
\n
• Establish effective follow-up procedures to monitor patient progress and adapt treatment strategies as necessary.
\n
• Create relapse prevention plans and support patients in maintaining their mental stability and well-being.
\n
• Employ strategies to identify "at-risk" patients and proactively develop intervention plans to avert potential deterioration of their conditions.

• Maintain thorough and accurate documentation of patient progress, treatment outcomes, and relevant diagnostic information.
\n
• Foster communication with primary care providers to ensure holistic care coordination and promote integrated treatment plans.
\n
• Equip patients with self-management skills and educational resources, empowering them to take an active role in their recovery journey.
\n
• Contribute insights regarding behavioral health services to clinical management and assist in evaluating program outcomes.

• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals, ensuring the highest level of ethical treatment and care.

• Perform other duties as assigned to support clinic operations and patient care.

Patient Population / Client Focus\n

This role will engage with a diverse patient population, including veterans and civilians facing an array of emotional and mental health challenges. You will cater to individuals requiring support for depression, anxiety, PTSD, and other mental health concerns, guiding them through their recovery pathways while providing compassionate care that is sensitive to their unique backgrounds and experiences.

Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field is required.

• Must hold a current Licensed Professional Counselor (LPC) certification.

• Proven knowledge of mental health assessments, treatment planning, developmental psychopathology, family therapy, and behavioral therapy methodologies.

• Ability to communicate in both English and Spanish is highly preferred.
\n
• Strong interpersonal skills and the mental and physical health needed to meet the demands of the role.

• Extensive knowledge of how mental and physical health issues impact individuals, families, and communities.

• Familiarity with community resources that can aid in patient treatment and recovery.

• Previous experience in a telehealth setting is advantageous.
