Key Responsibilities\n

• Conduct comprehensive assessments to identify the needs and goals of each individual client, ensuring a tailored approach to treatment.

• Engage in effective communication and collaboration with supervisors, care team members, family members, and other stakeholders to monitor and discuss client progress.

• Develop and maintain a structured schedule for client appointments, progress evaluations, and necessary home visits tailored to specific client needs.

• Coordinate home visits as necessary to observe client behaviors in their natural environments, ensuring holistic treatment strategies.

• Implement program plans effectively, making necessary adjustments based on ongoing assessments and client feedback.

• Participate actively in Interdisciplinary Team Meetings to discuss treatment strategies and ensure coordinated care.

• Prepare and submit detailed monthly and quarterly progress notes, keeping all documentation up-to-date and accessible.

• Organize training sessions to enhance the competencies of staff regarding program plans and documentation requirements.

• Provide ongoing follow-up interviews and support through home visits to reinforce client goals and adapt care plans as necessary.

• Maintain accurate and comprehensive records to ensure transparency and continuity of care.

• Serve as a liaison between clients, their families, and the broader mental health community to promote awareness and accessibility of our services.

Qualifications\n

• A Master’s degree or Doctorate in Counseling, Psychology, or a related field is required.

• Possession of active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Arizona.

• Expertise in family therapy, couples counseling, substance abuse, or trauma-informed care is highly desired to align with our client needs.

Compensation and Benefits\n
