Work Environment\n

This role is performed remotely, allowing for flexibility while maintaining productivity. While working independently, counselors will engage with clients through virtual appointments, facilitating their care and ensuring that therapeutic goals are met effectively. Our outpatient office serves as a hub for collaboration, where LPCs work closely with primary care providers and other health professionals to create an integrated care approach for our patients.

Key Responsibilities\n

• Work collaboratively with primary care providers to identify, assess, and manage the mental health needs of clients, contributing to an integrated care team.

• Conduct comprehensive patient intakes, focusing on diagnostic assessments, functional evaluations, and making informed recommendations regarding treatment plans.

• Provide brief, focused interventions for clients requiring immediate mental health support, based on their individual needs.
\n
• Facilitate referrals to specialty mental health services when necessary, ensuring clients receive the appropriate level of care.
\n
• Initiate regular follow-ups to monitor clients' progress and adapt treatment plans as required for optimal outcomes.
\n
• Develop and implement relapse prevention plans that assist clients in maintaining stability and managing their conditions effectively.
\n
• Identify at-risk clients and collaborate on strategies to prevent deterioration of their mental health.
\n
• Document progress, diagnoses, and treatment plans diligently in clients' charts, ensuring compliance with agency guidelines.
\n
• Engage in proactive discussions with primary care providers regarding clients’ treatment, needs, and optimal interventions.
\n
• Empower clients by providing self-management skills and educational resources to facilitate their active participation in recovery.
\n
• Offer behavioral health consultations to clinic management and team members, promoting program evaluation and outcome measures.
\n
• Assist clients in navigating medical treatments suggested by their primary care providers, helping them manage any side effects or challenges.

• Uphold the highest standards of ethical conduct outlined in the Code of Ethical Conduct for Clinical Professionals.

• Perform other related duties as assigned by clinic leadership.

Qualifications\n

• A Master's Degree in Clinical Counseling is required.

• Must possess a current Licensed Professional Counselor (LPC) license.

• Familiarity with DSM-IV diagnoses and a solid grounding in theories of behavioral therapy.

• Strong communication skills and the ability to collaborate effectively with a multi-disciplinary team.

• General mental and physical health sufficient to meet the demands of the position.

Preferred Experience\n

• Experience working with a veteran population or underrepresented communities is a plus.

• Bi-lingual proficiency in English and Spanish is preferred, although not required.

• Previous experience in an outpatient setting or integrating mental health care within primary care is advantageous.

Compensation and Benefits\n

The annual salary for this position ranges from USD 87,000 to 105,000, commensurate with experience and qualifications.
