Key Responsibilities\n

• Act as an integral part of the primary care team by collaborating with physicians and healthcare providers to address mental health and psychosocial issues for patients.

• Conduct thorough patient assessments, focusing on diagnostic evaluations to inform treatment plans in collaboration with the primary care provider.

• Offer targeted interventions, ensuring patients receive appropriate mental health services tailored to their needs.

• Guide primary care providers in making informed decisions about patient referrals to specialized mental health services when necessary.

• Initiate follow-up procedures to monitor patient progress, adjusting treatment strategies based on evolving needs.

• Develop comprehensive relapse prevention plans to help patients navigate their mental health challenges effectively.

• Identify at-risk patients and proactively formulate preventative strategies to mitigate potential deterioration of their mental health conditions.

• Coordinate behavioral health services beyond the clinic, with patient consent, to ensure comprehensive care.

• Maintain accurate and detailed records of patient progress and diagnostic information in the treatment chart.
\n
• Establish transparent communication channels with primary care providers regarding patient needs and treatment plans.

• Assist patients in adhering to medical treatments and navigating potential medication side effects while promoting healthy lifestyle choices.

• Comply with the Code of Ethical Conduct for Clinical Professionals in all interactions and documentation.

• Perform other duties as assigned to ensure the smooth operation of the clinic.

Qualifications\n

Compensation and Benefits\n

The position offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
