Key Responsibilities\n

• Conduct ongoing evaluations of clients to monitor progress and adapt treatment plans as required.

• Facilitate communication among stakeholders, including families, guardians, and multidisciplinary team members, to ensure cohesive care.

• Develop and share a detailed care plan with supervisors, highlighting appointments, home visits, and clinical observations.
\n
• Organize and perform home visits to conduct in-home observations and review patient charts, ensuring a holistic approach to each client's care.
\n
• Ensure the effective execution of the program strategy, reinforcing treatment methods that align with best practices in mental health.
\n
• Participate in Interdisciplinary Team Meetings, contributing insights and collaborating on patient care decisions.
\n
• Complete monthly and quarterly progress notes to document client development and facilitate ongoing care.
\n
• Coordinate training sessions for staff members to enhance their effectiveness in delivering mental health programs and proper documentation.
\n
• Engage in follow-up interviews, home visits, and evaluations to address changing needs and ensure continuity of care.
\n
• Maintain accurate and up-to-date records for easy access by team members and external stakeholders.

Patient Population / Client Focus\n

Your role will involve working closely with a diverse array of clients, including both adults and youth, allowing you to apply specialized therapeutic techniques. You will be instrumental in promoting mental well-being by providing counseling services tailored to the unique needs of each client.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is mandatory.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Colorado is required.

• A minimum of two years of counseling experience, preferably within a clinical setting, is essential.

• Strong knowledge of legal and ethical standards in mental health practice.

• Exceptional interpersonal and communication skills, with an ability to build rapport with clients from diverse backgrounds.

• Demonstrated ability to work collaboratively within a healthcare team.

• Proficiency in using Electronic Medical Records (EMR) software and related technology is a plus.
