Description

• Conduct thorough evaluations of patients in either an inpatient or outpatient setting and develop tailored treatment plans
• Implement physical therapy interventions with a focus on improving mobility, strength, balance, and functional ability
• Collaborate closely with nursing staff, physicians, case managers, and rehabilitation professionals to achieve optimal patient outcomes
• Guide patients and their families on therapeutic objectives, discharge plans, and home exercise regimens
• Maintain accurate documentation of evaluations, treatments, and patient progress according to facility and regulatory standards
• Engage actively in care team rounds and interdisciplinary meetings to guarantee coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
