Catholic Guardian Services in Bronx, NY is seeking a Case Planner for the Family Connections program. You will identify appropriate treatment or service providers and coordinate referrals to internal and external resources, while evaluating clients to assess risks and develop case plans. Requirements include a BA/BSW degree and child welfare experience. Bilingual Spanish is a plus.
