Role Description

The Housing Care Coordinator is a full-time, on-site role based in Tulsa, OK. This position involves coordinating housing services for clients, conducting assessments to determine individual needs, and developing client-specific housing and care plans. The Housing Care Coordinator will collaborate with clients, service providers, and community resources to ensure effective case management and provide necessary support. Additional responsibilities include maintaining accurate documentation, conducting home visits, and advocating for clients to facilitate access to vital services and resources.

Primary Responsibilities:

• Complies with the agency, housing, and care coordination program policies and procedures as established by Tulsa CARES.
• Adheres to the Oklahoma State Department of Health Standards of HIV Case Management Care.
• Adheres to HOPWA requirements established by the Oklahoma Housing Finance Agency (OHFA) and Housing and Urban Development (HUD).
• Supports an environment of health and wellness in accordance with the agency’s philosophy of care.
• Works as part of an interdisciplinary case-management, nutrition, housing, and mental health team to meet the needs of the entire person.
• Maintains a work environment that is conducive to effectively providing services to clients in a private and confidential manner.
• Works closely with other programs to provide a continuum of health care and to promote coordination and collaboration with other health and/or social service organizations.
• Ensures that all mandatory documentation of services is completed in a timely manner and all data reports reflect services rendered.
• Minimal travel required.

Qualifications

Education & Experience

• Bachelor's degree from an accredited university in social work, human services, or a related field preferred; equivalent combination of education and relevant experience may be considered.
• Must maintain current driver’s license and any relevant licenses or certifications.
• At least two years of experience in housing navigation, case management, care coordination, supportive housing, homelessness services, HIV services, behavioral health, or related direct-service work is preferred.
• Experience working with those living in poverty, with HIV/AIDS, with other chronic health conditions, and/or with history of mental health or substance abuse is preferred.

Knowledge, Skills, & Abilities

• Strong organizational skills with the ability to prioritize and address multiple tasks in a fast-paced, client-centered environment
• Skilled in ethical decision-making and culturally responsive service delivery; experience with motivational interviewing preferred
• Excellent written and verbal communication skills with the ability to collaborate effectively across teams and with diverse community partners.
• Proficient in Microsoft Office Suite (Word, Excel, Outlook) and electronic documentation systems with strong attention to accurate and timely recordkeeping.
• Bilingual skills, particularly Spanish-English interpretation and communication, strongly preferred.

• While performing the duties of this job, the position requires lifting and/or move up to 20 pounds, kneeling, stooping and/or standing for longer periods of time, bending, turning, reaching, talking, and listening.
• Prolonged periods sitting at a desk and working on a computer.
• Have the hand-eye coordination and manual dexterity needed to operate a keyboard, photocopier, telephone, fax machine, and other office equipment
• Have a normal range of hearing and eyesight to record, prepare and communicate appropriate reports; specific vision requirements: the ability to focus.
• Must be able to travel with dependable transportation.
