Senior Case Manager: Community Rehab & Housing Lead

ACMH, Inc. in Queens seeks a Senior Case Manager/Case Manager to provide case coordination and deliver community rehabilitation services at the Queens Treatment Apartment (QTA). The role reports to the Program Director and works on a Mon-Fri 9:00am–5:00pm schedule with a focus on recovery, housing support, and integrated care. The position requires a Master's or min. 3 years direct care experience in human services, strong communication skills, and a dedication to serving adults with serious #J-18808-Ljbffr ACMH, Inc.
