· Connect participants to appropriate community resources, including recovery supports, housing, public benefits, health and behavioral health care, transportation, legal assistance, childcare, and other essential services.
· Coordinate employment services, including career assessment, job-readiness support, résumé and interview preparation, referrals to employers, and connections to occupational training, education, credentialing, and apprenticeship opportunities.
· Maintain regular contact with participants and service partners; monitor progress toward plan goals; identify emerging barriers; and adjust interventions and referrals as needed.
· Lead case conferences for complex or high-need situations and coordinate services with treatment providers, peer specialists, workforce staff, employers, training providers, and community organizations.
· Apply motivational interviewing, trauma-responsive practices, harm-reduction principles, and culturally responsive approaches while maintaining professional boundaries and participant confidentiality.
· Respond appropriately to participant crises, safety concerns, relapse-related needs, and changes in level of care in accordance with organizational policies, mandated reporting requirements, and emergency protocols.
· Complete timely and accurate case notes, assessments, service plans, referrals, releases, progress updates, and outcome documentation in required databases and files.
· Support continuous quality improvement by reviewing documentation, tracking participant milestones, identifying service gaps, sharing resource information, and contributing to program reports and partner meetings.
REQUIREMENTS:
· Bachelor’s degree in social work, counseling, psychology, human services, criminal justice, public health, or a related field required. A relevant combination of education, professional experience, and recognized credentials may be considered in accordance with organizational policy.
· At least three years of progressively responsible case management experience serving adults with substance use, behavioral health, reentry, homelessness, or other complex service needs; experience coordinating complex cases is strongly preferred.
· Demonstrated knowledge of recovery-oriented systems of care, community reentry, workforce development, public benefits, housing resources, and community referral networks.
· Experience developing individualized service plans, documenting participant progress, coordinating multidisciplinary services, and using participant data to support outcomes.
· Strong engagement, assessment, crisis-response, advocacy, organization, written communication, and interpersonal skills.
· Ability to work effectively with people from varied backgrounds, maintain confidentiality and professional boundaries, and use a trauma-responsive, person-centered approach.
· Proficiency with Microsoft Office and electronic case-management or client-tracking systems; ability to maintain accurate records and meet documentation deadlines.
· Ability to travel locally to community partners and service locations as needed. Valid driver’s license and access to reliable transportation may be required based on program operations.
