Registered Nurse, Start of Care

Admissions Nurse

The Admissions Nurse's primary focus is to perform the initial SOC (Start of Care) visit although other visit types may be assigned based on agency needs. Responsibilities include determination of appropriateness for home care, overall assessment, medication reconciliation, development of the plan of care and provision of direct care and treatments as ordered. All documentation related to the SOC is completed within agency timelines and with Quality Standards in collaboration with and under the guidance of a Clinical Field Supervisor.

Responsibilities include:

• Ensure complete and accurate assessment and delivery of skilled nursing services.
• Determine appropriateness of patient for home care.
• Develop patient centered plan of care based on comprehensive assessment.
• Develop a plan of care using interdisciplinary approach.
• Ensure that all documentation meets quality standards.
• Understand and follow nurse code of ethics and standards of practice.
• Coordinate with internal and external resources to meet patient needs.
• Maintain established productivity expectations.
• Communicate client care issues to interdisciplinary team including physician.
• Identify needs and refer to additional services as indicated to meet patient goals.
• Report findings and plans to designated RN Case Manager/Care Team and/or Clinical Supervisor.
• Work in conjunction with Supervisor and designated staff to efficiently manage patients using a team approach.
• Supervise home health aides in performing designated tasks related to patient care and per care plan.
• Communicate appropriate information to home health aide department.
• Maintain confidentiality.
• Demonstrate knowledge and use of infection control practices.
• Demonstrate knowledge of federal, state and local government rules and regulations.
• Demonstrate knowledge of agency policies and procedures.
• Adhere to quality improvement standards.
• Keep personnel requirements up-to-date; i.e., health requirements, licensure requirements, and mandatory in-services.
• Demonstrate proficiency with skills competency through periodic evaluations.
• Participate in educational in-services and meetings.
• Maintain and update clinical knowledge on an ongoing basis.
• Represent self and agency in a professional manner including compliance with agency dress code.
• Other duties as assigned.

Requirements include:

• Current New York State registered professional nurse license.
• Degree from an accredited school of nursing or college.
• Current and valid driver's license and auto insurance, and a vehicle which can be used for work.
• Self-directed and organized with the ability to work independently.
• Good oral and written communication skills.
• OASIS Certification, or willingness to obtain within 1 year of hire.
• Minimum one year experience in Home Health Care required.

Physical Requirements:

• Ability to sit or stand for long periods during work day
• Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 50 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.
• Ability to drive in ALL weather conditions (rain, sleet, snow, wind).
• Visual/hearing ability sufficient to comprehend written or verbal communication
• Ability to use telephone and communicate over the phone
• Frequently uses a laptop computer to enter orders and obtain product information
• Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
