Registered Nurse - Field Assessor

Job Title We are hiring for a PRN RN Field Assessor to join our passionate team in Ridgefield, CT! Responsibilities Complete Long Term Care insurance assessments, including obtaining claimant demographics, physician information, and a current medication list. Evaluate the claimant’s functional level of independence and assess the ability to complete ADL and IADL tasks. Perform a brief cognitive screening as part of the assessment process. Use a point‑of‑care web application/system on a laptop or tablet for real‑time documentation. Required Qualifications Current and unrestricted RN licensure in the state of practice. Current CPR certification. Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation. Proven ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. Preferred Qualifications 1+ years of RN experience. Proven ability to work independently. Proven good communication, writing, and organizational skills. Pay Target Pay: $82,995 - $124,492 annually. Hourly Pay Range: $39.90 - $59.85. Per Visit Point Pay Range: $47.88 - $71.82. Pay is based on local labor markets, education, work experience, certifications, and other factors.
