Manager - Case Management

Role Summary: Manages daily operations of the Care Coordination department, overseeing RN Case Managers, Social Workers, and administrative staff. Responsibilities include developing departmental goals aligned with strategic vision, assessing patient care quality, and ensuring effective communication among staff, physicians, and departments. Provides coaching, mentoring, and ensures staff compliance with policies. Identifies process gaps, collaborates on improvements, and monitors budgets, regulatory compliance, and vendor relations. Manages staff performance, recruitment, scheduling, and payroll, applying evidence-based practices to deliver patient-centered care. Requires a Bachelor\'s in Nursing or Master\'s in Social Work, with five years of experience including two in leadership. Must hold relevant state licensure (RN, LCSW, LMSW) depending on location.
