Responsibilities:

• Conduct thorough assessments of new residents, including medical and social histories, to develop individualized care plans

• Collaborate with interdisciplinary teams to ensure coordinated care and communicate effectively with families and caregivers

• Develop and implement effective admission and transfer protocols to ensure a smooth transition for residents

• Provide education and support to residents and their families on medication management, disease prevention, and overall wellness

• Participate in Quality Improvement initiatives to ensure high-quality care and patient satisfaction

• Maintain accurate and up-to-date records and reports

• Demonstrate a commitment to person-centered care and a passion for working with older adults

• Other duties as assigned by DON or designee
