Community Health WorkerLocation: Remote (Redding area) Salary: $50,000 - $62,400/year Schedule: Monday-Friday, standard business hours (occasional evenings/weekends as needed)About the RoleWe're seeking an experienced Community Health Worker to provide care coordination and support services to underserved populations. You'll work with individuals enrolled in enhanced care management programs, helping them navigate healthcare, housing, and social services while addressing barriers like homelessness, chronic illness, and behavioral health challenges.Key ResponsibilitiesConduct community outreach and educate people about available healthcare and social servicesEnroll eligible members into managed health plansManage a caseload and coordinate referrals from health plansNavigate members through healthcare, housing, behavioral health, and social service systemsConduct home and community visits to assess needs and provide health educationConnect members with appropriate medical, housing, behavioral health, and community resourcesAdvocate for individuals facing complex physical, behavioral, or social challengesUse motivational interviewing to encourage positive behavior change and self-advocacyCollaborate with healthcare providers, social workers, health plans, and community organizationsMaintain accurate, timely, and confidential documentationCollect and report required dataRequired QualificationsMinimum 2 years of professional experience in community health, outreach, care coordination, or related fieldDemonstrated experience enrolling members into managed health plansStrong communication, cultural sensitivity, advocacy, and documentation skillsAbility to conduct home visits and travel locally throughout the assigned countyMust live within or near the assigned service areaPreferred QualificationsBachelor's degree in Social Work, Psychology, Sociology, or related disciplineCommunity Health Worker certificationKnowledge of healthcare systems and medical terminologyFamiliarity with Medi-Cal, Medicare, CalAIM, or similar programsExperience supporting individuals experiencing homelessness, behavioral health conditions, substance use disorders, or complex social needsBenefitsMedical, dental, and vision insurancePaid vacation and sick timePaid holidaysRetirement planEqual Opportunity Employer: Decisions are made without regard to race, religion, color, national origin, sex, gender identity, sexual orientation, disability, age, veteran status, or other legally protected status.
