Key Responsibilities\n

As a Licensed Clinical Social Worker at Elite Care Therapy Solutions, you will:

• Provide virtual or telephonic educational support to patients, caregivers, and educational staff to enhance understanding of mental health resources and treatment pathways.

• Act as the primary point of contact for your patients and collaborate with other healthcare professionals to ensure continuity of care.

• Assess and understand each patient’s unique support needs and preferences to facilitate a personalized experience that promotes adherence to treatment plans.

• Educate patients regarding program policies and maintain thorough documentation in clinical charts, ensuring compliance with regulatory standards.

• Keep accurate records pertaining to client progress, update progress notes regularly, and ensure the clinical chart contains necessary and current information on the patient’s mental health and treatment journey.

Patient Population / Client Focus\n

Your primary focus will be on school-aged children and adolescents, including their families. You will engage with a diverse population, providing culturally competent support and addressing a range of mental health challenges, such as anxiety, depression, and behavioral issues that may impact their educational experience. Qualifications\n

The ideal candidate should possess:
