Key Responsibilities\n

• Provide individual and group counseling sessions focusing on issues such as grief, adjustment to illness, and coping mechanisms to aid patients and families.

• Coordinate with community organizations to arrange post-discharge services that foster continuity of care and support patients' reintegration into their home environments.

• Offer education and support to families regarding available treatment options and community resources to improve overall understanding and cooperation in care processes.

• Participate in hospital committees to share expertise on social work-related matters and contribute to policy development.

• Provide crisis intervention, including immediate counseling and emotional support, during critical situations faced by patients and their families.
