Work Environment\n

This role is uniquely positioned within a telehealth framework allowing for flexibility in your work schedule. While core hours are Monday to Friday, 8:00 AM to 5:00 PM, occasional evening shifts may be required based on patient demands. Each day, you will have access to advanced telehealth tools to conduct sessions and maintain patient records securely and efficiently.

Key Responsibilities\n

• Conduct comprehensive mental health assessments that include detailed evaluations of how patients’ medical conditions impact their mental well-being.

• Design and implement integrated treatment plans that address both psychiatric and medical therapeutic needs, utilizing evidence-based frameworks.

Patient Population / Client Focus\n

You will primarily work with a diverse patient population, including individuals with chronic illnesses, substance use disorders, and other significant mental health issues. Qualifications\n

• A Master’s or Doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.

• Current licensure as an LCSW, LCPC, or LMFT in the state of Illinois.

• Demonstrated proficiency in treating individuals with co-occurring medical and mental health conditions.
\n
• Experience in integrated care settings, collaborating with medical professionals to optimize patient care.

• Strong interpersonal and communication skills with the ability to maintain professional boundaries and patient confidentiality.

• Familiarity with electronic health record (EHR) systems, as well as other digital communication and telehealth platforms.
