Key Responsibilities\n

As a Licensed Professional Counselor, your daily responsibilities will include:

• Conducting thorough assessments to develop tailored care plans based on patients’ unique needs and mental health conditions.
\n
• Facilitating communication among supervisors, direct care staff, family members, and other stakeholders on client progress and needs.
\n
• Creating and providing a schedule for supervisory appointments, home visits, and progress observations to track and promote effective care.
\n
• Coordinating home visits to conduct wellness checks, gather insights for chart reviews, and ensure comprehensive service delivery.
\n
• Monitoring and ensuring that treatment plans are implemented effectively, making necessary adjustments as required.
\n
• Participating in Interdisciplinary Team Meetings to collaborate on case management and treatment strategies.
\n
• Completing detailed monthly and quarterly progress notes to document client development and treatment efficacy.
\n
• Organizing training sessions for staff to enhance their capacity to implement program plans effectively and maintain accurate documentation.
\n
• Providing ongoing support through follow-up interviews, home visits, and observations to ensure continued progress towards clients’ goals.
\n
• Maintaining comprehensive and accurate records, ensuring that pertinent information is readily accessible to authorized parties.
\n
• Acting as a liaison between our organization, clients, and the broader mental health community to foster supportive relationships and development initiatives.

• Assisting in the creation of procedural documentation for training materials geared toward improving our services and client programs.

Patient Population / Client Focus\n

In this role, you will engage with a diverse population, including adults and youth dealing with various mental health issues. Your expertise will support individuals facing challenges such as anxiety, depression, trauma, and substance abuse. By fostering a safe and welcoming space, you will empower clients to navigate their personal journeys towards recovery and well-being.

Qualifications\n

To be successful in this role, candidates must meet the following requirements:

• Master's or doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted LPC licensure in the state of Tennessee.
\n
• A minimum of two years of counseling experience, preferably in different clinical settings.
\n
• Strong understanding of ethical and legal standards in healthcare.
\n
• Excellent interpersonal and communication skills, capable of building rapport with diverse clientele.
\n
• Proven ability to work collaboratively with a multidisciplinary healthcare team.
\n
• Proficiency with electronic medical record (EMR) software and other relevant technologies.

Preferred Experience\n

While not mandatory, the following experiences are highly desirable:

• Specialization in areas such as family therapy, couples counseling, trauma-informed care, or substance misuse treatment.

Compensation and Benefits\n

We offer a competitive salary range of USD 90,000 – 100,000 per year, commensurate with experience and available caseload. Additionally, our benefits package includes:
