Key Responsibilities\n

As an LCSW at our Hospice Center, your responsibilities will include:

Patient Population / Client Focus\n

Your caseload will primarily consist of patients diagnosed with terminal conditions, including cancer, heart disease, and neurodegenerative diseases. Alongside these patients, you will also support their families, providing counseling and resources to help them navigate the emotional and logistical challenges that accompany end-of-life care. Your work will be fundamental in facilitating family meetings, advanced care planning, and bereavement support, fostering a holistic environment for healing.
