Registered Nurse Assessment Coordinator – RNAC - MDS RN

We are seeking to hire a  Registered Nurse Assessment Coordinator – RNAC to become a part of our facility’s compassionate care-giving team.

• $3,000 Sign-On Bonus*
• $40 to $43 per hour based on experience.
• 3 days/week; Day Shift (Wednesdays are mandatory); All shifts are on-site.

Registered Nurse Assessment Coordinator – RNAC   Qualifications:

• Licensed as a Registered Nurse - RN in Pennsylvania.
• Minimum of 1 year of MDS experience.
• Minimum of 1 year of Long-Term Care experience.
• Minimum of 1 year of Care Plan experience.

Registered Nurse Assessment Coordinator – RNAC Introduction:

The RNAC assists with accurate and timely completion of both the RAI and Care Management process from admission to discharge.

Registered Nurse Assessment Coordinator – RNAC Qualifications Job Summary:

• Complete all required sections of the MDS.
• Intensive focus on Care Plans:  creation, documentation, monitoring and leading the Care Plan meetings.
• Ensure documentation is present in the medical record to support MDS coding.
• Work in collaboration with the IDT to assess the needs and formulate a plan of care for the residents.
• Obtain and ensure Skilled Certification/Re-Certification documentation is accurate.
