We CARE for our patients like they are our own FAMILY. ## **The Christ Hospital Home Health Care Monday–Friday | 8:15 AM–5:00 PM****Bring Compassionate Care Home!**Join The Christ Hospital Home Health Care team and make a meaningful impact by providing skilled nursing care to patients in the comfort of their own homes.As a Home Health LPN, you’ll provide one-on-one patient care, support recovery, educate patients and families, and work alongside a collaborative clinical team focused on positive patient outcomes.**Why Join Us?** Monday–Friday schedule Guaranteed base hourly rate Uncapped bonus opportunities Mileage reimbursement Medical, dental, and vision insurance Paid time off 401(k) participation Company-paid life insurance Employee Assistance Program Support from scheduling, admissions, and clinical teams**What You’ll Do*** Provide skilled nursing care in patients’ homes under physician orders* Follow care plans and goals created by the RN Case Manager* Monitor patient conditions and report changes* Administer medications and treatments as prescribed* Educate patients and families on disease management and preventative care* Help reduce re-hospitalizations through patient education and proactive care* Complete clinical documentation in accordance with agency and Medicare guidelines* Participate in on-call, case conferences, trainings, and required meetings**Qualifications*** Current Licensed Practical Nurse license in the state of employment* Minimum of 1 year of LPN experience in an acute care setting required* 2+ years preferred* Home health experience preferred* Valid driver’s license and active auto insurance* Strong communication, time management, and documentation skills* Compassionate, dependable, and patient-focused***Make a Difference Every Day****If you’re an LPN looking for meaningful one-on-one patient care, autonomy in the field, and the support of a strong clinical team, we’d love to connect with you.*#INDCHRI7We’ll help you put your passion for patient care to work.
