Inpatient Palliative Care Social Worker - Full Time

Summary The palliative care social worker is responsible for providing and documenting individualized, person-centered psychosocial assessment, advance care planning, discharge planning, and ongoing emotional support for individuals with serious illnesses and their families within an acute care hospital setting. The role collaborates with various medical teams and ensures continuity of care and a medical plan aligned with patient and family goals of care. Responsibilities Work collaboratively with an interdisciplinary palliative care consultation team of physicians, advance practice providers, chaplains, and learners to provide supportive care to patients with serious illnesses and their families. Effectively communicate and coordinate with interdisciplinary members of other medical teams to ensure continuity of care and facilitate complex discharge planning. Complete psychosocial assessments focused on patient and family coping with serious illness and advance care planning. Provide ongoing emotional support and psychoeducation to patients and families regarding disease progression, death/dying process, and advance care planning. Facilitate completion of advance directive documents when appropriate. Co-facilitate family meetings to identify and address goals of care. Document in the electronic medical record in a timely manner using appropriate palliative care psychosocial assessment, advance care planning, and discharge planning tools. Maintain relationships with palliative care and hospice community agencies and stay up-to-date on resources and laws related to advance care planning. Plan, facilitate and participate in palliative care educational programs for interdisciplinary staff within the hospital and across the health system. Credentials Social Worker (Required) Advanced Certified Hospice-Palliative Social Worker (Required) Education or Equivalent Experience Master of Arts or Science (Required) Salary and Benefits Salary Range: $64,390.56 - $102,664.22 per annum based on 40 hours per week. As part of the job offer, successful candidates receive a specific rate based on experience and education.
