Key Responsibilities\n

• Conduct comprehensive assessments of clients’ mental health to identify emotional distress and formulate individualized treatment plans tailored to their specific situations and goals.

• Facilitate individual and group therapy sessions using evidence-based therapeutic approaches to foster personal growth and healing.

• Collaborate with a multidisciplinary team, including social workers, medical professionals, and hospice staff, to coordinate holistic care and support the mental health needs of patients and their families.

• Educate family members about mental health issues, treatment options, and coping strategies, enhancing their involvement in the care process.

• Maintain meticulous and confidential patient records, adhering strictly to ethical and regulatory standards to ensure compliance with health care guidelines.

Patient Population / Client Focus\n

You will primarily work with patients facing terminal illnesses, along with their families, addressing a range of mental health issues, including grief, anxiety, depression, and adjustment disorders. Your expertise in empathy and active listening will be crucial as you provide a therapeutic presence during their most vulnerable moments.

Qualifications\n

• A valid and current license as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or a related mental health license.

• A Bachelor’s degree in Social Work, Psychology, Counseling, or a closely related field is required.

• At least 2 years of direct experience in mental health counseling, preferably in a hospice or end-of-life care setting.

• Demonstrated knowledge of therapeutic modalities applicable to grief and loss, as well as emotional support strategies.

• Exceptional interpersonal skills, with the ability to build rapport with clients and families quickly and effectively.

Preferred Experience\n

• Previous experience working in hospice care or with terminally ill patients will be highly regarded.
