What You'll Do

• Screen, evaluate, and rehabilitate patients with physical functioning disorders or wounds, following all clinical standards
• Develop and adjust patient care plans using standardized assessments and evidence-based practices
• Administer therapies including manual techniques, ambulation, therapeutic exercises, modalities, and wound care
• Monitor patient progress and modify treatments accordingly; document outcomes thoroughly in electronic records
• Provide consultation and education to patients, families, and caregivers
• Collaborate with healthcare team members to ensure coordinated care and optimal outcomes
• Support supervising and guiding physical therapist assistants, rehab aides, and students
• Participate in care conferences, meetings, training sessions, and compliance activities
• Maintain all documentation in adherence to regulatory and organizational standards

What You Need

• Valid state license as a Physical Therapist
• Experience in Long Term Care preferred
• Strong communication and clinical skills
• Ability to work independently and as part of a team
• Adherence to APTA guidelines, ethical standards, and state practice acts
