Key Responsibilities

• Conduct thorough health assessments, including comprehensive physical exams and detailed patient histories, to establish accurate diagnoses and create effective treatment plans.
• Order and interpret diagnostic tests such as lab work and imaging studies, providing timely and evidence-based interventions to patients in need.
• Prescribe medications and perform in-home therapeutic procedures, addressing acute and chronic health issues directly within the patient’s residence.
• Educate patients and their families on health promotion strategies, disease prevention techniques, and the management of ongoing health conditions to empower them in their own care.
• Collaborate with interdisciplinary team members to coordinate patient care and facilitate timely referrals to specialists as necessary, ensuring seamless continuity of care.
• Maintain accurate, up-to-date medical records in compliance with regulatory standards, utilizing electronic health record systems to enhance the quality of patient interactions.

Qualifications

Compensation and Benefits
