Work Environment

This role involves working onsite in various community settings within New York City. Key Responsibilities

• Conduct comprehensive health assessments and physical examinations to identify patient needs, develop diagnoses, and create tailored treatment plans.
• Utilize established standardized protocols alongside independent clinical judgment to manage acute and chronic health conditions.
• Educate patients and families on health promotion, disease prevention strategies, and lifestyle modifications to improve overall health and wellness.
• Collaborate closely with a multidisciplinary team of healthcare professionals, sharing insights and treatment options to enhance patient care.
• Maintain accurate and up-to-date patient records using electronic health record (EHR) systems, tracking evidence of patient progress and treatment efficacy.
• Participate in quality improvement initiatives aimed at optimizing care delivery and improving patient outcomes within the organization.

Qualifications

• A Master’s degree in Nursing from an accredited institution.

Compensation and Benefits

• Competitive annual salary ranging from USD 150,000 to 160,000, commensurate with education and experience.
