Work Environment\nThis role is entirely remote, giving you the flexibility to work from your home while remaining connected to our compassionate team. Our hospice center serves a diverse patient population in Elizabeth, New Jersey, addressing both psychological and emotional issues related to terminal illnesses. You will be part of an integrated care approach, collaborating closely with other healthcare professionals to ensure holistic patient support.
Key Responsibilities\n- \n
- Provide individual psychotherapy and mental health services to patients experiencing conditions such as anxiety, depression, or post-traumatic stress disorder (PTSD) within a hospice care context. \n
- Conduct comprehensive assessments of patients’ mental health to evaluate emotional, behavioral, and social functioning, ensuring a thorough understanding of their unique needs. \n
- Develop, implement, and regularly update treatment plans that are evidence-based, culturally competent, and personalized to fit each patient’s circumstances and treatment goals. \n
- Engage in continuous monitoring of patient progress, adapting treatment plans as necessary based on patient responses and feedback from interdisciplinary team members. \n
- Educate patients and their families about mental health conditions, treatment options, and available community resources, supporting them in navigating their care journey. \n
- Ensure adherence to ethical, regulatory, and professional standards, including maintaining patient confidentiality and securing informed consent. \n
The patients you will serve include individuals with serious, life-limiting illnesses in a hospice setting. This vulnerable population often grapples not only with physical symptoms but also emotional and psychological burdens. Your role will be integral in addressing these multifaceted challenges, providing a safe space for expression, and fostering coping mechanisms to enhance their quality of life in terminal stages.
Qualifications\n- \n
- A Master’s degree in Social Work from an accredited institution. \n
- Current state licensure as a Licensed Clinical Social Worker (LCSW) is required. \n
- A minimum of 2 years’ experience providing psychotherapy and mental health services, with a focus on patients coping with significant health conditions being a strong advantage. \n
- Exceptional communication and interpersonal skills to effectively build rapport with patients and work collaboratively within a healthcare team. \n
- Proficient in conducting assessments, developing effective treatment plans, and applying clinical skills in diverse situations. \n
- Knowledgeable in utilizing electronic health records (EHRs) and other digital tools to maintain accurate patient documentation and coordinate care. \n
- Familiarity with various mental health conditions and a deep understanding of the psychological needs of patients facing terminal illnesses. \n

- Experience in hospice or palliative care settings is preferred. \n
- Previous roles that required interdisciplinary collaboration with healthcare teams, especially those involving physicians and nurses. \n
- Background in family therapy or group therapy modalities would be viewed positively. \n
