Key Responsibilities\n

• Conduct thorough assessments of patients’ social and emotional needs, providing individualized interventions that contribute to their overall treatment plans.

• Collaborate closely with an interdisciplinary treatment team to facilitate coordinated care and develop effective discharge plans.

• Document all patient interactions and progress in electronic health records promptly and accurately to ensure continuity of care.

• Foster positive relationships with patients, families, and healthcare team members to build trust and support successful patient outcomes.

Patient Population / Client Focus\n

This role requires a focus on diverse patient populations that may include individuals facing a range of challenges such as mental health issues, emotional distress, trauma, and family dynamics. Understanding the unique social determinants of health affecting our clients in Sammamish and surrounding areas will be essential in delivering tailored interventions that lead to positive outcomes.

Qualifications\n

• A Master’s degree in Social Work from a CSWE-accredited institution is required.

• A current and valid Licensed Independent Clinical Social Worker (LICSW) license in the state of Washington.

• Compassionate ability to provide care in a clinical setting with patients from various backgrounds, demonstrating sensitivity and understanding.

• Basic Life Support (BLS) certification is mandatory.
\n

Preferred Experience\n

While a minimum of three years of clinical experience is required, candidates with additional experience in community health settings, crisis intervention, or specialized therapeutic practices will be highly regarded.
