This full-time position is ideal for licensed therapists who are looking to make a positive impact in the field of mental health while providing care in a remote setting at a hospice center. Work Environment\n

This role is predominantly remote, allowing for the flexibility needed to manage your schedule while serving a diverse patient population. As a part of our hospice center team, you will collaborate with an array of other health professionals from the comfort of your own home. The core work hours are Monday to Friday, from 9:00 AM to 5:00 PM, with some flexibility for client needs. You will engage in regular virtual team meetings and consultations to ensure cohesive care delivery.

Key Responsibilities\n

• Conduct thorough mental health assessments by utilizing structured diagnostic interviews to determine the needs and treatment goals of patients.

• Develop and implement personalized treatment plans that incorporate evidence-based interventions to support individual, couple, and family therapy sessions.

• Facilitate psychoeducational groups to address shared mental health concerns, promoting community engagement and emotional support.
\n
• Collaborate with psychiatrists and other healthcare professionals to enhance patient care and maintain continuity in treatment plans.

• Provide crisis intervention services during mental health emergencies, ensuring immediate support and safety for patients.
\n
• Educate patients and their families about mental health conditions, treatment options, and community resources available to them.
\n
• Maintain accurate and confidential records in compliance with HIPAA and other legal standards, utilizing electronic health record systems effectively.

• Participate actively in ongoing professional development opportunities, staying abreast of best practices and new advancements in mental health therapy.

• Contribute to case discussions and treatment team meetings, sharing insights and strategies that improve overall patient outcomes.

Patient Population / Client Focus\n

In this role, you will primarily serve patients experiencing complex emotional challenges, particularly in hospice settings where patients and families face terminal illness. Your efforts will not only address mental health needs but also provide essential support to enhance the quality of life during difficult transitions.

Qualifications\n

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current licensure as an LCSW, LPC, or LMFT in the state of New Jersey.
\n
• Demonstrated expertise in conducting comprehensive mental health assessments and developing effective treatment plans tailored to individual needs.

• In-depth knowledge of various evidence-based therapeutic modalities, particularly cognitive-behavioral therapy (CBT) and solution-focused therapy.

• Strong interpersonal and communication skills, with the ability to build rapport with diverse patient populations.

• Capacity for sound judgment, ethical practice, and the ability to maintain professional boundaries.

• Familiarity with electronic health records and telehealth technology.
\n
• Ability to work collaboratively in a team and contribute to a cohesive care environment.

• A minimum of 2 years of direct experience in providing mental health therapy, particularly in outpatient or hospice care settings.
