As a remote team member, you will primarily work from the comfort of your home, allowing for a flexible work-life balance. However, you will maintain a strong connection with our Hospice Center as you engage with clients both through telehealth platforms and occasionally in person. Key Responsibilities\n

• Assess clients through telephonic and in-person interactions, gathering comprehensive information to inform treatment planning.

• Provide critical crisis intervention services to clients experiencing acute emotional distress or risk of harm.
\n
• Maintain clear, accurate, and confidential records of client interactions and treatment plans in compliance with ethical and legal standards.
\n
• Facilitate client referrals to community agencies and services as well as connect clients and their families to resources that will aid recovery and resilience.
\n
• Engage clients in therapeutic activities and support them in accessing necessary services to enhance their overall well-being.
\n
• Attend and participate in training sessions, clinical supervision, and team meetings to foster professional growth and ensure the highest standard of care.

• Monitor for signs of child abuse in accordance with agency policies and relevant state laws to protect vulnerable populations.

Patient Population / Client Focus\n

You will primarily serve diverse populations affected by a variety of social and mental health challenges, including children and families dealing with domestic violence, child abuse, substance abuse, and mental health disorders. A strong sense of empathy and the ability to recognize the complex reactions of clients facing these issues are essential for this role. Qualifications\n

• A Master's degree in Social Work (MSW) or a related field is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Colorado.
