Job Responsibilities

• Conduct patient assessments in inpatient or outpatient settings and develop personalized treatment regimens
• Administer physical therapy interventions concentrating on mobility, strength, balance, and overall function
• Collaborate effectively with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Guide patients and their families on treatment objectives, discharge plans, and home-based exercises
• Document assessments, treatments, and progress in line with facility and regulatory standards
• Engage in care team rounds and multidisciplinary conferences to ensure cohesive care

Education

• Doctorate in Physical Therapy (DPT) or a relevant degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., neurological, geriatric) are an asset but not obligatory

Skills

• Prior experience strongly preferred
