Description

• Conduct assessments for patients in both inpatient and outpatient settings and create tailored treatment strategies
• Implement physical therapy interventions aimed at enhancing mobility, strength, and functional abilities
• Work in collaboration with nursing staff, physicians, case managers, and other rehabilitation specialists to optimize patient outcomes
• Provide education to patients and their families regarding therapeutic goals and discharge plans alongside home exercise protocols
• Document evaluations, treatments, and progress meticulously following all facility and regulatory standards
• Engage in interdisciplinary care team meetings to facilitate seamless patient care

Education

• Doctoral degree in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Prior experience in a similar role is strongly preferred
