Per Diem Physician – In-Home Health Evaluations Signify Health, part of CVS Health®, is seeking a Per Diem Physician to provide in‑home health evaluations. The role involves visiting patients in their private residences, conducting medication & medical history reviews, physical evaluations, and, when ordered, diagnostic tests such as diabetic eye exams or spirometry. Responsibilities Conduct 45‑minute in‑home visits, connecting one‑on‑one with patients Review medication and medical history and perform physical evaluations Order and conduct appropriate diagnostic and preventive services (e.g., spirometry, DEE) Provide all Signify Health services as indicated, including virtual visits as needed Travel locally and within licensed states as required Maintain accurate clinical documentation using the iPad workflow Qualifications MD or DO with an active, unrestricted license in the coverage area Multi‑state licenses preferred Ability to perform duties in diverse home environments, including those with pets, secondhand smoke, or other allergens Experience with in‑home or community health evaluations is a plus Compensation and Benefits Hourly base rate of $105.00, plus incentives per completed visit and per diagnostic service Malpractice insurance coverage during Signify Health‑related activities Eligible for benefits such as dental, vision, wellness resources, and employee discounts (depending on weekly hours) Flexible part‑time engagement with anticipated weekly hours of 12 Eligibility for benefits will be determined based on hours worked and program eligibility.
