Registered Nurse - Case Manager

Employment Type: Full-Time

Location: Palm Springs, CA

Compensation: $44 – $54 per hour

As a pivotal member of the interdisciplinary care team, the RN Case Manager identifies the physical, psychological, social, and spiritual needs of patients and their families. Beginning at admission, you will initiate targeted interventions and design a comprehensive, responsive, and continuously updated plan of care to provide exceptional support during critical life stages.

Responsibilities:

• Serve as the central point of contact to align services across the care team, including social workers, chaplains, home health aides, volunteers, and physicians.
• Provide direct skilled nursing care as dictated by the treatment plan. Regularly update care plans to reflect changes in patient condition and integrate quality improvement metrics.
• Conduct medication reconciliation at every change in level of care, updating both the electronic health record (EHR) and the hospice pharmacy. Ensure the appropriate and documented use of pharmaceuticals, supplies, and durable medical equipment (DME).
• Perform on-site performance evaluations for Certified Home Health Aides (CHHAs) and volunteers. Act as a preceptor for nursing students or assist with new team member orientation as needed.
• Appropriately utilize and integrate the expertise of contracted ancillary personnel, including Physical Therapy (PT), Occupational Therapy (OT), Speech Therapy, Dietary, and Pharmacy.
• Actively participate in Interdisciplinary Team Meetings, manage the insurance re-certification process, and maintain complete, accurate, and timely documentation.
• Maintain a highly collaborative and positive professional demeanor. Provide patients and colleagues with clear communication regarding your availability, and participate in a rotational on-call, weekend, and holiday schedule as required.

Requirements:

• Current, active California RN License.
• Minimum of 1 year of professional nursing experience with a Diploma, ASN, or BSN (a BSN may substitute for the 1 year of required work experience). Prior experience in home health or hospice care is highly preferred.
• Strong foundational understanding of the principles and practices of primary medical and nursing care, alongside sharp critical thinking and independent problem-solving skills.
• Ability to communicate effectively and assertively with individuals at all levels of the organization, as well as patients, families, and outside providers.
• Valid California driver’s license with proof of auto insurance. BLS certification required within 60 days of hire.

Physical Demands & Work Environment:

• Desk and field-balanced role involving sitting (60%), standing (20%), and walking (20%).
• Requires frequent reaching above the head (25%) and occasional lifting of equipment/supplies up to 40 lbs.
• Highly technical workspace utilizing computers, cell phones, and tablets for more than 4 hours per day
