Key Responsibilities\n

• Conduct comprehensive assessments to define the needs and goals of individual clients, ensuring personalized care plans are developed.

• Maintain clear and open communication with supervisors and team members regarding client progress, treatment adjustments, and appropriate interventions.

• Provide timely scheduling of appointments, both virtual and in-person, ensuring a structured approach to monitoring client progress.

• Coordinate home visits and community-based support when necessary, which may include conducting chart reviews, staff interviews, and data analysis.

• Ensure successful implementation of personalized treatment plans while actively participating in multidisciplinary team meetings to discuss holistic approaches to care.

• Document client progress through detailed monthly and quarterly notes, capturing essential insights that inform future treatment decisions.

• Lead training sessions designed to ensure staff competency in executing treatment protocols and proper documentation practices.

• Deliver ongoing support through follow-up interviews and observations, reinforcing skills learned to support clients' growth and resilience.

• Compile and maintain organized records to facilitate information accessibility for all stakeholders involved in the treatment process.
