Key Responsibilities\n

• Act as a liaison between clients, families, and outpatient providers, ensuring seamless communication and coordination of care.

• Engage with insurance companies to facilitate pre-certification and manage utilization effectively to minimize barriers to treatment.

• Participate actively in discharge planning and aftercare strategies to ensure continuity of care for clients transitioning out of intensive services.

• Provide therapeutic meal support, guiding clients in developing healthy relationships with food while reinforcing positive coping mechanisms.

• Deliver empathetic and affirming therapy sessions that honor the unique identities of each client and address their specific challenges.

Patient Population / Client Focus\n

In this role, you will engage with a diverse range of clients, including older adults navigating complex emotional and psychological challenges. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• A valid LCSW license in New York is necessary to practice.

Compensation and Benefits\n
