Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate patients’ emotional, social, and environmental factors impacting their recovery through interviews, observations, and review of medical records.

• Provide empathetic counseling and support to patients and their families, helping them navigate adjustment issues and enhancing their emotional well-being throughout the rehabilitation process.

• Collaborate with a multidisciplinary rehabilitation team to develop and implement individualized treatment plans that integrate the patients’ psychosocial needs, ensuring a holistic approach to care.

• Assist patients in accessing essential community resources, including support groups, vocational rehabilitation services, and housing assistance, to facilitate a smooth transition back into their communities.

• Educate patients and families about available community resources and post-rehabilitation services, empowering them with knowledge and tools for continued support.

• Participate actively in discharge planning meetings to ensure continuity of care and effective follow-up post-rehabilitation.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

• Current and valid California license as a Licensed Clinical Social Worker (LCSW).

• At least one year of relevant clinical experience, preferably in a healthcare or rehabilitation setting.
