Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, integrating emotional, mental, and social factors into individualized care plans.

• Work directly with clients and their families to develop, implement, and adapt personalized care plans that reflect their goals and preferences.
\n
• Provide counseling and emotional support, assisting seniors and their families in navigating life transitions, such as health changes or the loss of loved ones.
\n
• Advocate for clients by ensuring their rights are upheld and that they have access to necessary community resources and support services.
\n
• Collaborate effectively with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to enhance the overall care provided to clients.

• Regularly monitor clients’ well-being through virtual visits, assessing living conditions, and addressing any concerns regarding safety and care.

• Educate clients and their families about available community resources, support groups, and programs that can enhance their social interactions and overall quality of life.

• Maintain accurate and confidential documentation of client assessments, care plans, and interactions using electronic health record (EHR) systems.

• Remain informed on industry best practices and ethical standards to ensure compliance with pertinent regulations.

Patient Population / Client Focus\n

As a Social Worker at Gotham Enterprises, you will primarily work with seniors facing various health challenges. Your expertise will be critical in evaluating their psychosocial needs and implementing care strategies that support their physical, emotional, and social wellness. Families will also be a focal point of your work, as you guide them through complex emotional landscapes and advocate for their loved ones’ best interests.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Independent Clinical Social Worker (LICSW).

• A minimum of two years of experience in a social work capacity, preferably within a community or hospice setting.

• Strong knowledge of social work theories, aging-related issues, and community resources available for seniors.
\n
• Proficient assessment and counseling skills, with the ability to empathize and communicate effectively with seniors and their families.

• Excellent organizational skills and proficiency in EHR and other healthcare-related technologies.

Preferred Experience\n

• Experience providing support in hospice or palliative care settings is highly desirable.

• Demonstrated capability in facilitating family meetings or mediation in complex family dynamics related to caregiving.

Compensation and Benefits\n
