Key Responsibilities\n

• Conduct thorough assessments to identify and address clients' social, emotional, and psychological needs as they adjust to end-of-life care.

• Provide empathetic counseling and support to individuals and families understanding their challenges, grief, and transition processes.

• Deliver both individual and group counseling sessions utilizing evidence-based therapeutic approaches to foster healing and resilience.

• Develop and implement personalized service plans that align with client goals, ensuring responsiveness and adaptability in approaches.

• Advocate passionately for clients’ rights, facilitating access to essential resources and addressing social justice issues encountered.

• Engage actively in program development and evaluation, contributing to grant reporting to ensure sustainability and growth of our initiatives.

• Collaborate with a wide range of community stakeholders, government agencies, and volunteers, enhancing service delivery and outreach efforts.

• Participate in regular team meetings, ongoing training, and community outreach activities to continuously inspire and educate.

Patient Population / Client Focus\n

Your primary focus will be on patients receiving hospice care and their families, guiding them through one of life’s most challenging transitions. You will provide emotional and psychological support tailored to the unique needs of patients nearing the end of life. This role requires a compassionate approach to address grief, fear, and the complex emotions that arise during this time.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) is a must.

Compensation and Benefits\n
