Regional Admissions Liaison Skilled Nursing Facility
Job Summary:
Prestige Healthcare Services is seeking a highly motivated and organized Regional Admissions Liaison to join our team at our skilled nursing facilities in Richmond, VA. As a Regional Admissions Liaison, you will play a crucial role in ensuring seamless admissions processes for our short-term and long-term care patients. Responsibilities:
Coordinate and facilitate the admissions process for patients transitioning from acute care settings to our skilled nursing facility
Collaborate with healthcare providers and families to ensure a smooth transition for patients
Develop and maintain relationships with local healthcare facilities, hospitals, and physicians to generate referrals
Conduct assessments and screenings to identify patient needs and develop personalized care plans
Maintain accurate records and reports of admissions, transfers, and discharges
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Regional Admissions Liaison : Serve as a liaison between our facility and local healthcare providers, hospitals, and physicians to ensure effective communication and referral processes
Requirements:
Regional Admissions Liaison : Excellent communication, organizational, and problem-solving skills
A high school diploma or equivalent required; associates or bachelor's degree in a healthcare-related field preferred
