Work Environment

This role will be based in a home care setting across the Port Charlotte area, allowing for a flexible and personalized approach to patient treatment. As a physical therapist in our organization, you will work independently while collaborating with an interdisciplinary team of healthcare professionals. Your expertise will play a critical role in delivering effective rehabilitation services and improving quality of life for our clients.

Key Responsibilities

• Develop and implement individualized treatment plans designed to alleviate pain, restore functionality, and enhance mobility based on the patient's specific orthopedic needs.
• Supervise Physical Therapy Assistants to ensure adherence to the established plan of care, providing guidance and support during patient sessions.
• Conduct thorough initial evaluations and ongoing assessments using diagnostic tests to analyze muscle, nerve, joint, and functional abilities.
• Assess patients' home environments and recommend necessary adaptations or modifications to improve their independence and quality of living.
• Deliver therapy services as prescribed by physicians, including manual therapy techniques, therapeutic exercises, and other intervention strategies.
• Document clinical findings, progress notes, and updates in patients' records, ensuring compliance with legal and organizational standards.
• Participate actively in case conferences with the interdisciplinary team to review patient progress and modify treatment plans as needed.
• Provide education to patients and their families on physical therapy devices and their home exercise programs to ensure continuity of care.
• Maintain timely communication with physicians and case managers regarding changes in patients' conditions and therapy responses.
• Adapt treatment methods and activities to the evolving needs of patients and utilize community resources to support recovery goals.

Patient Population / Client Focus

In this role, you will primarily work with orthopedic patients, many of whom are recovering from surgery, injury, or degenerative conditions. Our diverse patient population includes individuals of all ages who require specialized rehabilitative care. You will play a vital role in enhancing their physical capabilities, ensuring they achieve their rehabilitation goals and regain their independence.

Qualifications

• A Bachelor's Degree in Physical Therapy or a Doctorate of Physical Therapy (DPT) from an accredited institution.
• State licensure as a Physical Therapist through the Physical Therapy Examining Committee.
• Minimum of two (2) years of clinical experience, particularly in an orthopedic setting.
• Current CPR certification to respond effectively in emergency situations.

Preferred Experience

• Experience in home healthcare settings.
• Familiarity with OASIS documentation and care plan development.
• Proficient in using therapy equipment and tools specific to orthopedic rehabilitation.
