Registered Nurse Case Manager

Triniti Home Health & Hospice is hiring a RN Case Manager to join our team. This full‑time, benefits‑eligible position is based at our South Metro Branch in Edina. Position Details Position Type: Full-Time, benefits‑eligible Location: 4600 W. 77th Street, Edina, MN Wage Range: $42 – $45 per hour, depending on experience Responsibilities Assess the needs of home care patients, develop and implement personalized care plans, and execute interventions to help patients meet goals. Promote restoration of health through teaching, counseling, and preventive, rehabilitative, and palliative measures. Coordinate, oversee, and provide patient‑related care. Document and maintain paperwork, referral records, and reports. Display a courteous attitude and respect for all patients, families, and staff. Perform other duties as assigned. Qualifications Current MN Registered Nurse (RN) license required. 1+ year of prior RN experience required. Proficiency with MS Word, Excel, and Outlook. Strong written and oral communication skills and ability to collaborate with care team members. Ability to organize and prioritize work efficiently to accomplish tasks timely.
