Key Responsibilities\n

• Conduct comprehensive assessments to identify clients' social, emotional, and environmental needs, utilizing tools and methodologies aligned with best practices in social work.

• Develop individualized service plans in collaboration with clients, incorporating specific goals, interventions, and progress monitoring to ensure effective support.

Your primary focus will be on providing comprehensive social work services to individuals and families experiencing a range of challenges, particularly in end-of-life care situations within the hospice setting. You will be instrumental in offering emotional, psychological, and practical support to clients and their families navigating the complex emotional terrain of grief, loss, and transition.

Qualifications\n

• A Master's degree in Social Work from an accredited institution, demonstrating a solid foundation in social work practice.

• Possession of an active state license as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
