Key Responsibilities\n

• Conduct thorough assessments to identify clients' social, emotional, and environmental challenges, employing various evaluation methods to determine their needs.

• Develop and implement individualized service plans that define realistic goals, tailored interventions, and measurable progress indicators.

• Provide one-on-one counseling sessions to individuals and families, utilizing evidence-based therapeutic strategies to assist clients in navigating life’s challenges.

• Engage in crisis intervention techniques and safety planning when necessary, ensuring clients receive immediate support during critical situations.

• Collaborate with community agencies and stakeholders to coordinate services, advocate for clients' needs, and facilitate connections to available resources.

• Participate in community outreach initiatives aimed at raising awareness about available services and promoting health equity.

• Take part in multidisciplinary team meetings, case conferences, and ongoing training to remain informed about best practices in social work.

Patient Population / Client Focus\n

Your work will primarily involve serving individuals and families grappling with a variety of social challenges, including but not limited to homelessness, domestic violence, substance abuse, mental health issues, and poverty. By fostering trust and understanding, you help clients articulate their needs and empower them to access targeted support services that can lead to improved outcomes and greater self-sufficiency.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is a fundamental requirement.

• Possession of an active state license as a Licensed Clinical Social Worker (LCSW) is necessary to practice.

Compensation and Benefits\n
