Position Overview We are seeking a motivated and compassionate Physical Therapist (PT) to deliver high-quality in-home therapy services to patients throughout our California service areas. In this role, you will help patients improve mobility, restore function, reduce pain, and regain independence through personalized treatment plans. Your work will play a vital role in helping patients achieve safer, healthier, and more active lives at home. Key Responsibilities Conduct comprehensive physical therapy evaluations in patients’ homes Develop individualized treatment plans based on patient needs and physician orders Provide therapeutic exercises, mobility training, and pain management interventions Educate patients and caregivers on proper movement, fall prevention, and home safety Monitor patient progress and adjust treatment plans as needed Complete accurate and timely clinical documentation in compliance with regulations Collaborate with physicians, nurses, and interdisciplinary care team members Ensure compliance with Medicare, state, and agency policies Participate in care planning meetings and case conferences as required Qualifications and Requirements Current Physical Therapist license in the State of California Graduate of an accredited Physical Therapy program Minimum of one year of home health experience preferred (new graduates may be considered) Strong clinical assessment and rehabilitation skills Excellent communication and interpersonal abilities Ability to work independently and manage caseload efficiently Reliable transportation and valid driver’s license Current CPR certification Experience with electronic medical records (EMR) systems preferred Why Join InTouch Home Care Market competitive per-visit compensation Flexible scheduling and work-life balance Supportive clinical leadership and administrative team Steady patient referrals Opportunities for professional growth and continuing education Collaborative and positive work culture How to Apply Interested candidates are encouraged to submit their resume and credentials.
