Integrated Case Manager - Population Health

This is a 13-week contract position.

Shift: 8am-4:30pm with weekend rotation.

The Integrated Case Manager for Population Health is a key member of the patient-centered care team responsible for collaborative assessment, planning, facilitation, care coordination, evaluation, and advocacy for patients' healthcare needs. This role addresses the needs of patients experiencing critical events or diagnoses requiring complex management strategies to optimize health outcomes across the care continuum.

Responsibilities

• Conduct comprehensive assessments of patients and families to gauge biomedical, psychological, social, and functional needs.
• Develop personalized patient-centered care plans to enhance patient care experiences.
• Engage patients and their families through advocacy, ongoing communication, and service facilitation.
• Utilize critical thinking and motivational interviewing to assist patients in overcoming barriers to achieving their goals.
• Coordinate safe transitions of care between settings as necessary.
• Advocate for appropriate service delivery within health plan structures.
• Facilitate care conferences and collaborative management of complex cases.
• Evaluate patient responses to care plans and modify when necessary.
• Participate in process improvement activities, including data collection and analysis.
• Facilitate interventions for cases involving abuse and neglect.

Required Experience / Certifications / Licensure

• 5+ years of case management experience with 2+ years of recent acute inpatient case management experience.
• BSN or MSW degree is required.
• Current Michigan RN licensure or LMSW is mandatory.
• Experience in discharge planning is essential.
• Exceptional verbal and written communication skills.
• Strong problem-solving, analytical, and decision-making abilities.
• Knowledge of preventive service guidelines and case management principles.
