Work Environment

This role is based onsite in a home care setting, allowing for a personalized approach to therapy in the comfort of our patients' homes. You will work Monday to Friday, with a typical work schedule spanning 40 hours per week. Key Responsibilities

As an Orthopedic Home Care Physical Therapist, you will:

• Evaluate, design, and implement individualized treatment plans for patients referred from physicians, making adjustments based on patient progress.
• Utilize advanced therapeutic techniques tailored to the specific needs of each patient, reinforcing best practices in orthopedic, pediatric, neurologic, and sports medicine.
• Provide exceptional customer service, ensuring a positive experience while contributing to the overall efficacy of treatment programs.
• Collaborate with interdisciplinary teams to enhance patient outcomes through integrated, evidence-based care.
• Deliver skilled therapeutic interventions in patients' homes, employing modalities and techniques aligned with physician directives to achieve improved functional status.
• Educate patients and their caregivers, empowering them with knowledge and skills for effective self-management and recovery.
• Adhere to organizational goals, achieving minimum utilization targets while fulfilling additional business objectives.

Patient Population / Client Focus

Your role targets a wide range of patients, including those recovering from orthopedic surgical procedures, chronic pain management, and sports-related injuries. You will engage with diverse demographic groups, adapting treatment approaches to meet distinct needs, ensuring each patient feels respected and cared for.

Qualifications

We are looking for candidates who possess:

• A Doctorate degree in Physical Therapy (DPT) and a valid State License in Physical Therapy.
• A minimum of two years of clinical experience in physical therapy, with a focus on orthopedic care.
• A Basic Life Support Certification accredited by the American Heart Association.
• Strong written and verbal communication skills to facilitate effective interactions with patients, families, and team members.
• Proficient word processing and computer skills for documentation and patient management.
• Ability to cultivate supportive relationships with patients to help them surpass their functional goals.
• A passion for connecting positively with people, respecting their unique journeys and challenges.

Preferred Experience

We value candidates who have experience working within a home health setting or have previously engaged with diverse populations. Familiarity with electronic documentation systems or experience in a multidisciplinary team environment will be advantageous.
