Work Environment

The Home Care Orthopedic Physical Therapist role is primarily onsite, allowing you to develop strong relationships with clients in their familiar surroundings. Key Responsibilities

As a Home Care Orthopedic Physical Therapist, your responsibilities will include:

• Assuming leadership for directing physical therapy treatment aimed at preventing disability and relieving pain, while developing and restoring muscle function in accordance with individualized treatment plans.
• Supervising and mentoring physical therapy assistants to ensure compliance with care plans and adherence to best practices.
• Conducting thorough evaluations of patients' functional capabilities using diagnostic tests to inform treatment strategies.
• Adapting treatment interventions based on an analysis of clients' home environments to promote safety, accessibility, and independence.
• Delivering prescribed physical therapy services, which may involve administering manual therapy techniques, instructing patients in therapeutic exercises, and utilizing physical modalities such as massage, traction, and other agents for effective pain management.
• Collaborating with the healthcare team to develop, review, and update individual care plans as needed and participating in interdisciplinary case conferences.
• Maintaining accurate patient records, including initial evaluations, progress notes, and discharge summaries, and ensuring timely submission as per organization policies.
• Providing educational support to patients and families regarding physical therapy programs, equipment, and self-management techniques.
• Conducting regular assessments and adapting treatment plans in response to patient progress and feedback, while communicating changes to physicians and case managers.
• Staying accessible for consultation to support physical therapy assistants during treatments.

Patient Population / Client Focus

Your work will primarily involve clients needing orthopedic rehabilitation in their own homes. These patients may include individuals recovering from surgeries, managing chronic conditions, or seeking assistance in restoring mobility and function after injuries. Your ability to engage with diverse populations while tailoring approaches to each individual's clinical and emotional needs will be critical to their recovery journeys.

Qualifications

Candidates should meet the following qualifications:

• A Doctorate in Physical Therapy from an accredited institution.
• A valid physical therapy license in good standing within the state of New Jersey.
• Current CPR certification.
• A valid driver's license and access to reliable transportation.

Compensation and Benefits
